Facial rejuvenation through a facelift

A facelift is a cosmetic surgery procedure used to combat the signs of ageing. It aims to restore a youthful look to the patient by reducing the sagging which takes place on the skin of the cheeks and jawline, whilst also fighting against other changes which occur to one’s face with age.  

In some cases, a neck lift (platysmaplasty) is also performed as part of a face-lift to reduce fat deposits and sagging skin on the neck.

Surgical Plan

The main steps to the procedure are infiltrating the layers of the face with saline solution and local anaesthetic making an inconspicuous incision, separating the facial skin; layers of subcutaneous fat and muscle, tightening the muscle, repositioning or removing the fat and removing excess skin. Operating techniques vary between surgeons; however, most modern cosmetic surgeons would offer their patients a variety of lifting techniques and choose the most appropriate for the patient.

The face is divided into three unequal areas. The brow, midface and lower face including the neck. The most popular facelift techniques deal with the lower third and may incorporate the other two.

Facelift Techniques

• S Lift and S Lift Plus – the name describes the shape of the incision which follows the shape of the ear and appears to be placed along the edge of cartilage rather than on the skin of the face itself. This method offers a relatively scar-free result. The “S Lift” re-tensions only the lower face and jaw. When the neck is included, the incisions are also placed behind the ear cartilage, and this technique is known as “S Lift Plus”.
• Mini Lift – this incision type is the same as the S Lift/S Lift Plus and aims to tighten excess or loose skin in the midface, and provide improvement to the lines from the corner of the nose to the mouth.
• Super Lift – the popular name for SMAS surgery where the muscle is tightened before the skin is re-tensioned. The mid-face, lower face and neck are all rejuvenated, and the incision continues from behind the ear and into the hairline at the back of the neck.
• Brow Lifting – Endoscopic, through a style of keyhole surgery. Temporal, minimising the incisions to 3cm placed inside the hairline. Full brow, the incision site is placed in the hairline across the top of the forehead.
• Baby face – the smoothing and rounding of the face by repositioning or even adding to the fat across the cheek. The procedure can be in conjunction with a mid-facelift or as a maintenance procedure some years after a facelift.

Most facial aesthetic surgeries are now offered to patients as a day only surgical procedure; although some elect to stay in the hospital until bandages are removed at 1 to 2 days postoperatively. 

Advances in cosmetic procedures of this type include the introduction of hydra dissection which employs water instead of a scalpel to separate tissues. This technique almost certainly reduces the risk of postoperative bleeding, minimises bruising and swelling and eliminates the need for uncomfortable drain tubes.
